چکیده

As the frequency of performing abdominal computed tomography (CT) scanning is increasing, owing to the increased quality of medical services, the prevalence of adrenal incidentalomas is also rising. Although most adrenal incidentalomas are benign and nonfunctioning, a substantial proportion demonstrates subtle hormonal hypersecretion. Adrenal adenomas mainly manifest as autonomous cortisol secretion, with a reported prevalence ranging from 1% to 47%, depending on the applied diagnostic criteria [1]. However, adrenal tumors secreting aldosterone and cortisol simultaneously are rare, especially among incidentalomas [2,3]. The recognition of cortisol and aldosterone cosecreting adrenal adenomas will have implications for the perioperative management, and is necessary for administering glucocorticoid treatment to avert the possibility of adrenal crisis [4]. Herein, we present a case of an incidentally detected adrenal adenoma secreting excess cortisol and aldosterone and discuss how this case was managed.

Secondary Hypertension: Diagnosis and Management of an Adrenal Adenoma

OBJECTIVES After participating in the continuing education activity, primary care physicians should be able to: 1. Know the renal and adrenal causes of secondary hypertension 2. Know the components of the clinical examination of a hypertensive patient 3. Be familiar with laboratory tests used to assess an incidentally discovered adrenal mass 4. Know the usual findings in primary aldosterone exc...

Analysis of mRNA expression for steroidogenic enzymes in the remaining adrenal cortices attached to adrenocortical adenomas.

DESIGN AND METHODS We have recently demonstrated that the adrenal cortices attached to aldosterone-producing adenoma (APA) contained microscopic subcapsular micronodules suggestive of active aldosterone production. In this study, we used in situ hybridization to investigate the mRNA expression of steroidogenic enzymes in the adrenal cortices attached to cortisol-producing adenoma (CPA) and clin...

A case of aldosterone-producing adrenocortical adenoma associated with a probable post-operative adrenal crisis: histopathological analyses of the adrenal gland.

We describe a case of aldosterone-producing adrenocortical adenoma (APA) associated with a probable post-operative adrenal crisis possibly due to subtle autonomous cortisol secretion. The patient was a 46-year-old female who suffered from severe hypertension and hypokalemia. CT and MRI scans revealed a 2-cm diameter adrenal mass. The patient's plasma aldosterone level was increased, and her pla...

Bilateral Adrenocortical Masses Producing Aldosterone and Cortisol Independently

A 31-year-old woman was referred to our hospital with symptoms of hypertension and bilateral adrenocortical masses with no feature of Cushing syndrome. The serum aldosterone/renin ratio was elevated and the saline loading test showed no suppression of the plasma aldosterone level, consistent with a diagnosis of primary hyperaldosteronism. Overnight and low-dose dexamethasone suppression tests s...

Adrenal Cortex Tumors and Hyperplasias

The adrenal cortex tumors include both malignant adrenal cortex cancers (ACC) and benign masses (ACT) that can be either secreting, of one of the hormones normally produced in the adrenal cortex (Cushing’s syndrome if the hypersecretion is of cortisol or Conn’s syndrome if it is aldosterone) or non-secretory (Incidentalomas). The outer part of the adrenal glands, the adrenal cortex, is responsi...
